Player Evaluation
Instructions
IMPORTANT: In order to keep the teams as balanced as possible it’s important for coaches to fill out Player Evaluations. These along with New Player Assessments are used to organize teams. Players (and parents) will not see these evaluations. They will only be seen by the Sports Department.
DIRECTIONS: Put the player’s name in the name area and type a score into each category. One (1) represents poor, five (5) represents average and ten (10) represents outstanding. Rate each player against the league as a whole. Do not be afraid to give low numbers. Then total the players rating numbers.
